Composition
1 capsule contains: loperamide hydrochloride 2 mg. Auxiliary substances: magnesium stearate, corn starch, microcrystalline cellulose.
Pharmacological effect
Antidiarrheal drug. Loperamide, by binding to the opioid receptors of the intestinal wall (modulating cholinergic and adrenergic neurons through guanine nucleotides), reduces the tone and motility of the smooth muscles of the intestine (by inhibiting the release of acetylcholine and prostaglandins). Slows peristalsis and increases the time of passage of intestinal contents. Increases the tone of the anal sphincter, helps to retain the feces and reduce the urge to defecate. The action comes quickly and lasts 4-6 hours.
Pharmacokinetics
Loperamide hydrochloride is almost completely metabolized by the liver by conjugation. T1 / 2 makes 9-14 hours. It is removed mainly with bile and urine in the form of conjugated metabolites.
Indications
Symptomatic treatment of acute and chronic diarrhea: - allergic, emotional, medicinal, radiation genesis; - with a change in diet and qualitative composition of food, in violation of metabolism and absorption; - as an aid in diarrhea of infectious genesis. Stool adjustment in patients with ileostomy.
Contraindications
- diverticulosis; - intestinal obstruction; - ulcerative colitis in the acute phase; - diarrhea on the background of acute pseudomembranous enterocolitis; - monotherapy for dysentery and other gastrointestinal infections; - I trimester of pregnancy; - lactation; - children under 6 years old; components of the drug. With caution, you should use the drug for liver failure.
Use during pregnancy and lactation
The use of the drug is contraindicated in the first trimester of pregnancy and during breastfeeding.
Dosage and administration
The drug should be taken orally with water. An adult with acute and chronic diarrhea is prescribed in an initial dose of 4 mg (2 pills or 2 capsules), then 2 mg (1 tablet or 1 capsules) after each defecation in the event of loose stools. The maximum daily dose is 16 mg (8 tab. Or 8 caps.). Children over the age of 6 years are prescribed 2 mg (1 tab.or 1 caps.) after each act of defecation in the case of loose stools. The maximum daily dose is 6 mg (3 tab. Or 3 caps.). With the normalization of the stool or the absence of stool for more than 12 hours, the treatment with the drug Diara should be stopped.
Side effects
On the part of the digestive system: gastralgia, dry mouth, intestinal colic, nausea, vomiting, constipation; very rarely - intestinal obstruction. From the side of the central nervous system: drowsiness, dizziness. Allergic reactions: skin rash.
Overdose
Symptoms: CNS depression (stupor, incoordination, drowsiness, miosis, muscle hypertension, respiratory depression), intestinal obstruction. Treatment: antidote - naloxone. Given that the duration of action of loperamide is longer than that of naloxone, repeated administration of the latter is possible. Symptomatic treatment: activated carbon, gastric lavage, mechanical ventilation. Medical supervision is required for at least 48 hours.
