Pharmacological effect
NSAIDs for topical use in ophthalmology. It has anti-inflammatory and analgesic action. The main mechanism of action of the drug is the inhibition of the synthesis of prostaglandins, which play an important role in the pathogenesis of inflammation and pain.
Pharmacokinetics
There is confirmed evidence that the drug penetrates into the anterior chamber of the eye. With a single instillation, the active ingredient of the drug is determined in the moisture of the anterior chamber for several hours. With topical application of Indocollir, systemic absorption is low.
Indications
- Inhibition of miosis during cataract surgery; - Treatment and prevention of inflammatory processes after surgical interventions on the eyeball; - Prevention of cystic edema of the macula after cataract operations; - Treatment of non-infectious conjunctivitis; - Treatment and prevention of post-traumatic inflammatory process during penetrating and non-penetrating injuries to the eyeball (as a supplement to local antimicrobial therapy).
Contraindications
- Hypersensitivity to the components of the drug; - Hypersensitivity to acetylsalicylic acid or other NSAIDs, manifested by attacks of bronchial asthma, the appearance of urticaria, acute rhinitis. With caution, Indocollir should be used for: - Epithelial herpetic keratitis (including a history); diseases that cause a violation of blood coagulation (including hemophilia, lengthening of the bleeding time, bleeding tendency).

Use during pregnancy and lactation
Currently there are no adequate and strictly controlled clinical studies on the safety of Indocollir during pregnancy and lactation (breastfeeding). However, the drug can be used in the first 5 months of pregnancy if the intended benefit of therapy for the mother outweighs the potential risk to the fetus. Starting from 6 months of pregnancy, you should not prescribe the drug, becausethis can lead to dysfunction of the vascular system and kidneys in the fetus. Indomethacin is excreted in breast milk, so if you need to use the drug during lactation, you should decide whether to stop breastfeeding or avoid using it during breastfeeding.
Dosage and administration
To inhibit intraoperative miosis, the drug is instilled in 1 drop in the conjunctival sac for 2 hours with an interval of 30 minutes (4 times) before surgery. To prevent cystic edema of the macula, the drug is instilled in 1 drop 3-4 times / day for 1 month after the operation. For other indications, 1 drop is prescribed 3-4 times / day, depending on the severity of the condition. The course of treatment can last from 1 to 4 weeks.
Side effects
On the part of the organ of vision: rarely - mild or moderate burning and / or temporary loss of clarity of vision immediately after instillation of eye drops. Local allergic reactions: rarely - itching and redness of the skin around the eyes. With long-term use of Indocollir, systemic side effects of indomethacin may develop.
Overdose
Accidental ingestion does not represent a risk of side effects.
Interaction with other drugs
Indocollir can enhance the effects of indirect anticoagulants, lithium preparations. It is not recommended to use this drug with diflunisal simultaneously (possible development of bleeding from the gastrointestinal tract) and other NSAIDs, including acetylsalicylic acid in high doses (3 g / day or more). Indomethacin with beta simultaneously. adrenergic blockers and saluretics can weaken the effects of the latter. If necessary, it can be used simultaneously with other eye drops, including containing GKS. At the same time, the interval between applications should be at least 5 minutes to prevent the leaching of the active substances with subsequent doses.
