Pharmacological effect
Antibiotic of the broad-spectrum aminoglycoside group. It has a bactericidal effect, disrupting protein synthesis and permeability of the cytoplasmic membrane of bacteria. Highly active against Pseudomonas aeruginosa. Cobramycin is active against gram-negative bacteria: Escherichia coli, Proteus spp. (indole-positive and indole-negative strains), Klebsiella spp., Serratia spp., Providencia spp., Enterobacter spp., Shigella spp., Salmonella spp., Citrobacter spp., Haemophilus influenzae; Gram-positive bacteria: Staphylococcus spp. (including penicillinase producing strains). Low active against Streptococcus spp. (including Enterococcus spp.).
Pharmacokinetics
When applied topically, systemic absorption is low.
Indications
For systemic use: severe infectious and inflammatory diseases caused by pathogens susceptible to tobramycin (sepsis, meningitis, peritonitis, endocarditis, respiratory infections, skin and soft tissue infections, urinary tract infections, bone infections). For local use: eye infections ( as part of combination therapy).
Contraindications
Hypersensitivity to tobramycin and other aminoglycosides.
Use during pregnancy and lactation
Tobramycin is possible when the intended benefit of treatment for the mother exceeds the risk of side effects in the fetus.
Dosage and administration
Individual, depending on the evidence, the severity of the disease, the patient's age. Introduce IM, IV drip Applied externally and topically in the appropriate dosage forms.
Side effects
On the part of the central nervous system and peripheral nervous system: ototoxic effect, headache, lethargy, disorientation, impaired neuromuscular transmission. On the part of the digestive system: nausea, vomiting, increased levels of hepatic transaminases and bilirubin in the blood. , thrombocytopenia. Local reactions: pains at the injection site. On the part of the metabolism: hypocalcemia, hyponatremia, hypokalemia, hypomagneemia. Others: nephrotoxic effect, allergic reactions are possible.
Overdose

Interaction with other drugs
With the combined use of tobramycin with furosemide and ethacrynic acid, an ototoxic effect may be enhanced; with other aminoglycosides, polypeptide antibiotics, some cephalosporins - may increase the nephrotoxic effect of tobramycin; with muscle relaxants - may increase their muscular-relaxing action.

Systemic use of tobramycin is possible when the expected benefit of treatment exceeds the risk of side effects in the following cases: in patients with impaired renal function, hearing, with myasthenia and parkinsonism.
