Pharmacological effect

It has a local anesthetic, adsorbing and enveloping effect, reduces the influence of damaging factors on the mucous membrane. Almagel A provides prolonged local neutralization of continuously separated gastric juice and reduces its hydrochloric acid content to optimal boundaries for treatment. Aluminum hydroxide suppresses pepsin secretion, neutralizes hydrochloric acid, forming aluminum chloride, which is converted into alkaline aluminum salts in the alkaline medium of the intestine. Magnesium hydroxide also neutralizes hydrochloric acid, turning into magnesium chloride. Thus, the effect of aluminum hydroxide, which causes constipation, is counteracted. Sorbitol, which is part of the preparation, promotes enhanced secretion of bile and exhibits a mild laxative effect, complementing the action of magnesium hydroxide. Almagel A does not sharply raise the pH of the contents of the stomach, buffers its value from 4.0–4.5 to 3.5–3.8 (physiological value) in the period between doses.The drug forms a protective layer, ensuring uniform distribution of active substances on the gastric mucosa and has a long-lasting local effect without subsequent formation of carbon dioxide in the stomach, which, in turn, causes flatulence, a feeling of heaviness in the epigastric region and secondary enhancement of hydrochloric acid secretion. According to the classification of Hodge and Sterner, the drug when administered orally is a weakly toxic agent and does not have embryotoxic, teratogenic and mutagenic effects. In some cases, increased tendon reflexes were observed in newborns whose mothers took the drug for a long time. In addition, in newborns there is a risk of hypermagnesia, especially in the state of dehydration, therefore prolonged use of the drug is not recommended for pregnant women and newborns.

Almagel A is a nonabsorbable drug.
