Pharmacological effect
Combined drug. Sodium Heparin is a direct anticoagulant, anti-inflammatory, anti-proliferative, anti-edematous and analgesic. Reduces platelet aggregation, binds to antithrombin III, preventing the transfer of prothrombin to thrombin. Inhibits thrombin activity. Reduces the activity of hyaluronidase, improves blood fibrinolytic properties. It improves microcirculation and activates tissue metabolism, thereby accelerating the resorption of hematomas and blood clots, ultimately restores the patency of the veins, clinically this is accompanied by a pronounced analgesic and anti-inflammatory effect. Dexpanthenol - provitamin B5 - in the skin turns into pantothenic acid, which is part of coenzyme A, which is involved in the processes of acetylation, carbohydrate and fat metabolism; stimulates skin regeneration, normalizes cellular metabolism, increases the strength of collagen fibers. It has a regenerating, vitamin, metabolic and anti-inflammatory effect. Troxerutin - flavonoid (a derivative of rutin), has P-vitamin activity; has a venotonic, angioprotective, anti-edema, antioxidant and anti-inflammatory effect. It participates in redox processes, blocks hyaluronidase, stabilizes hyaluronic acid of cell membranes and reduces capillary permeability and fragility, increases their tone. Increases the density of the vascular wall, reduces the exudation of the liquid part of the plasma and diapedesis of blood cells. Reduces inflammation in the vascular wall, limiting adhesion to its surface platelets. Benzocaine is a local anesthetic for surface anesthesia. Reduces the permeability of the cell membrane to sodium ions, displaces calcium ions from receptors located on the inner surface of the membrane, blocks the conduction of nerve impulses.It prevents the appearance of pain impulses in the endings of the sensory nerves and their passage through the nerve fibers. Benzocaine dilates the surface vessels, facilitating the absorption of heparin.
Pharmacokinetics
A small amount of heparin is absorbed from the skin surface into the systemic circulation. Cmax of the drug in the blood is noted 8 hours after application. The excretion of heparin mainly occurs through the kidneys, T1 / 2 for 12 hours. Troxerutin is well absorbed from the skin surface for external use. Dexpanthenol, when applied topically, is rapidly absorbed by the skin and converted into pantothenic acid. Its concentration in the blood - 0.5-1 mg / l, in the serum - 100 mcg / l. Pantothenic acid is not metabolized in the body (except for inclusion in Coenzyme A), it is excreted unchanged. Benzocaine with dermal application is practically not absorbed.
Indications

Contraindications

Use during pregnancy and lactation

Dosage and administration
Outwardly, apply a thin layer on the skin over the affected area and around it, evenly spreading over the skin surface with light circular movements 2-3 times / day. The course of treatment for 2-3 weeks, with recurrence with the relapse of the disease. In case of trophic ulcers with weak exudation, the wound surface is cleaned from exudate and necrotic tissues before use, if necessary, washed with a solution of hydrogen peroxide 3%, nitrofural 1: 5000 or chlorhexidine 0.05% and dried.The gel is applied with a uniform thin layer to the entire affected surface and a sterile gauze bandage is applied. Change dressings produce 1 time / day. With an open method of treatment, the drug is applied 1-2 times / day. The duration of treatment is determined by the dynamics of epithelialization. For subcutaneous hematomas, localized edema, aseptic infiltrates, injuries, bruises, and similar conditions, apply to the affected area 2-3 times / day until the damaged tissues are restored to normal condition.
Side effects
With long-term use of the drug, local reactions are possible in the form of skin hyperemia, decrease in sensitivity (at the site of application), allergic reactions (skin rash, itching).
Overdose
Cases of drug overdose Trombless Plus gel were not reported.
Interaction with other drugs
Combined use of the gel with oral anticoagulants can cause prolonged prothrombin time. Non-narcotic analgesics and anticholinesterase drugs enhance the action of benzocaine in the preparation. Benzocaine reduces the antibacterial activity of sulfonamides.
