Composition
Rectal suppositories 1 supp. heparin sodium 120 IU prednisone 1.7 mg lidocaine hydrochloride 20 mg. Excipients: 1 ml of p-ra d / and contains 100 mg (10 000 anti-Xa IU) of enoxaparin sodium.
Pharmacological effect
Pharmacological action is determined by the properties of the components included in the preparation. Heparin prevents the formation of blood clots, prevents coagulation of blood in hemorrhoids, has anti-edema action, promotes the regeneration of connective tissue, accelerates healing and reduces the number of relapses of the disease. Heparin inhibits blood coagulation and forms a fibrin clot. Heparin at this dosage in combination with plasma antitrombin III inhibits thrombosis, inactivating factor Xa, as well as inhibiting the conversion of prothrombin to thrombin. Heparin prevents the formation of a stable fibrin thrombus by inhibiting the activation of the fibrin-stabilizing factor. Heparin also has an anti-inflammatory effect, affecting the vascular endothelium, prevents edema. Prednisolone belongs to the group of synthetic glucocorticosteroids of medium duration of action, reduces the formation, release and activity of inflammatory mediators (histamine, kinin, prostaglandin, lysosomal enzymes). Suppresses cell migration to the site of inflammation. reduces vasodilation and increased vascular permeability in the inflammation. Reduces exudation due to vasoconstrictor action. Suppresses the action of macrophages, target cells, cytokines, which are involved in the development of allergic reactions. Prevents access of sensitized T-lymphocytes and macrophages to target cells. It has anti-inflammatory, anti-exudative and antiallergic effect, reduces inflammation and associated subjective sensations (itching, burning, pain, moisture due to eczematous changes) in the anorectal region. Lidocaine has a local anesthetic effect, which is caused by the blockade of voltage-dependent sodium channels, which prevents the generation of impulses at the end of the sensory nerves and the conduction of pain impulses along the nerve fibers.The combined effect of the constituent parts of the drug achieves the best effect.
Indications
- external and internal hemorrhoids, incl. sharp. - thrombophlebitis of hemorrhoidal veins of the perianal region. - fistula. - eczema. - Itching in the anus, caused by inflammation. - anal fissures. - preparation for surgical intervention in the anorectal zone. - as part of complex therapy of thrombosed and operated hemorrhoids.
Contraindications
- viral, fungal, tuberculosis and other infectious processes in the anorectal region, disorders of the coagulation system with a predisposition to bleeding - pregnancy and lactation period - children under 12 years old - hypersensitivity to the components of the drug.
Use during pregnancy and lactation
Use of the drug is contraindicated during pregnancy and lactation.
Dosage and administration
In case of internal hemorrhoids, suppositories should be introduced into the rectal cavity 1-2 times a day. In case of external hemorrhoids, the suppository should not be fully inserted into the anus - its base should be palpable outside. Hold the anus with a gauze cloth until the suppository is completely melted (2-3 minutes), otherwise the suppository will “fall” into the rectum without having a therapeutic effect. The course of treatment is 7 days. On the recommendation of the doctor, the course may be extended to 14 days.
Side effects
The drug is usually well tolerated. At the recommended dose of the drug, no side effects were identified. When using the drug for more than 5 weeks, systemic side effects associated with the action of prednisone are possible: On the part of the endocrine system: reduced glucose tolerance, "steroid" diabetes, adrenal suppression, Itching-Cushing syndrome. On the part of the cardiovascular system, arrhythmias, bradcardia (in predisposed patients), ECG changes characteristic of hypokalemia, increased blood pressure are possible. Of the nervous system: insomnia, dizziness. On the part of metabolism: increased excretion of calcium ions, hypocalcemia, increased perspiration. Allergic reactions caused by lidocaine and heparin in the form of itching and irritation of the anus are possible.
