Description

The solution for intravenous and intramuscular administration of Calcium gluconate-Vial compensates for the deficiency of calcium ions necessary to carry out the process of transmitting nerve impulses, reducing skeletal and smooth muscles, myocardial activity, formation of bone tissue, blood coagulation.

Active ingredients

Calcium gluconate

Release form

Ampoules

Composition

Calcium gluconate monohydrate, lactic acid, calcium hydroxide, water for injection.

Pharmacological effect

When administered intravenously, it stimulates the sympathetic part of the autonomic nervous system, enhances epinephrine release by the adrenal glands, and has a moderate diuretic effect.

Pharmacokinetics

After parenteral administration, the drug is evenly distributed with the blood stream in all tissues and organs. In plasma, about 45% is found in a complex with proteins. Passes through the placental barrier, passes into breast milk. Excreted mainly by the kidneys.

Indications

Diseases accompanied by: hypocalcemia, increased permeability of cell membranes (including vessels), impaired conduction of nerve impulses in muscle tissue. Hypoparathyroidism (latent tetany, osteoporosis), metabolic disorders of vitamin D: rickets (spasmophilia, osteomalacia), hyperphosphatemia in patients with chronic renal failure. Increased need for calcium (pregnancy, lactation period, the period of enhanced growth of the body), insufficient calcium content in food, impaired metabolism (in the postmenopausal period). Increased calcium excretion (prolonged bed rest, chronic diarrhea, secondary hypocalcemia on the background of long-term use of diuretics and antiepileptic drugs, glucocorticosteroids). Bleeding of various etiologies, allergic diseases (serum sickness, urticaria, febrile syndrome, pruritus, pruritic dermatosis, reactions to the administration of drugs and food intake, angioedema, edema, dystrophic alimentary edema, pulmonary tuberculosis, rickets, osteomalacia, lead colic eclampsia. Parenchymal hepatitis, toxic liver damage, nephritis, hyperkalemic form of paroxysmal myoplegia.

Contraindications

Hypersensitivity, hypercalcemia (calcium concentration should not exceed 12 mg or 6 mEq / l), severe hypercalciuria, nephroluritiasis (calcium), sarcoidosis, concomitant use of cardiac glycosides (risk of arrhythmias).

Precautionary measures

The drug should be used with caution in dehydration, electrolyte disturbances (risk of hypercalcemia), diarrhea, calcium nephrourolithiasis (history), minor hypercalciuria, moderate chronic renal failure, chronic circulatory insufficiency, common atherosclerosis, hypercoagulation, and the wykid during your work-life, during your workout day, your child will be exposed to an adult. the occurrence of necrosis).

Use during pregnancy and lactation

Allowed

Dosage and administration

Intramuscularly, intravenously slowly (within 2-3 minutes) or drip, for adults - 5-10 ml of 10% solution daily, every other day or after 2 days (depending on the nature of the disease and the patient's condition). Children - intravenously slowly (within 2-3 minutes) or drip, depending on age, from 1 to 5 ml of 10% solution every 2-3 days. The solution is heated to body temperature before administration. The syringe for the administration of calcium gluconate should not contain ethanol residues (to avoid precipitation of calcium gluconate). It is not recommended to administer the drug intramuscularly to children because of possible development of necrosis.

Side effects

With intramuscular and intravenous administration, nausea, vomiting, diarrhea, bradycardia may occur. When administered intravenously, there may be a burning sensation in the oral cavity, a sensation of heat, with rapid intravenous administration - a decrease in blood pressure, arrhythmia, fainting, cardiac arrest. With intramuscular administration, necrosis may form at the site of administration.

Overdose

Symptoms: hypercalcemia. Treatment: calcitonin, intravenously at the rate of 5-10 IU / kg bw / day (the drug is diluted in 500 ml of 0.9% sodium chloride solution, administered by drip for 6 hours).

Interaction with other drugs

Pharmaceutically incompatible with ethanol, carbonates, salicylates, sulphates (forms insoluble or hardly soluble calcium salts). Forms insoluble complexes with tetracycline antibiotics (reduces antibacterial effect).Reduces the effect of blockers of "slow" calcium channels (intravenous administration of calcium gluconate before and after verapamil reduces its hypotensive effect). With simultaneous use with quinidine may slow intraventricular conductivity and increase the toxicity of quinidine. During treatment with cardiac glycosides, parenteral administration of calcium gluconate is not recommended (it is possible that the cardiotoxic effect of glycosides may be enhanced). When combined with thiazide diuretics, it can enhance hypercalcemia, reduce the effect of calcitonin on hypercalcemia, and reduce the bioavailability of phenytoin.

special instructions

Patients with a slight hypercalciuria, a decrease in glomerular filtration, or with nephrorolithiasis in history, should be administered with caution and under the control of urinary calcium. To reduce the risk of nephrolurithiasis, it is recommended to drink plenty of fluids.
