Composition
1 tablet contains: Active substance: valaciclovir hydrochloride - 556.00 mg (in terms of valaciclovir 500.00 mg). Excipients: microcrystalline cellulose - 95.00 mg, hyprolose (hydroxypropylcellulose) - 14.00 mg, crospovidone - 28.00 mg, magnesium stearate - 7.00 mg. Film coating: [hypromellose - 10.50 mg, hyprolosis (hydroxypropylcellulose) - 4.07 mg, talc - 4.12 mg, titanium dioxide - 2.31 mg] or [dry mix for film coating - white, containing hypromellose (50 %), hydroxypropylcellulose (19.4%), talc (19.6%), titanium dioxide (11%)] - 21.00 mg.
Pharmacological effect
Acyclovir is an antiviral agent from the group of synthetic analogs of the acyclic purine nucleoside, deoxyguanidine, which is a component of DNA. Acyclovir is highly specific for Herpes simplex type 1 and type 2 viruses, the virus that causes varicella and shingles (Varicella zoster), Epstein-Barr virus. Moderately active against cytomegalovirus. Acyclovir penetrates directly into virus-infected cells. Cells infected with the virus produce viral thymidine kinase, which in turn phosphorylates acyclovir to acyclovir triphosphate, which is an active and selective inhibitor of viral DNA polymerase. Probably, the mechanism of inhibition of DNA synthesis by acyclovir triphosphate is that it is a substrate for this enzyme, allowing for the connection 3-5, necessary for the extension of the DNA chain. In this way, a premature break of the DNA chain is carried out, which leads to the suppression of viral replication.
Pharmacokinetics
When applying the ointment on intact skin: the absorption is minimal; not detected in blood and urine. Affected skin: moderate absorption.
Indications
Infections of the skin and mucous membranes caused by the Herpes simplex virus, genital herpes, shingles, chicken pox.
Contraindications
Hypersensitivity to acyclovir or to any other component of the drug.
Precautionary measures
Pregnancy, lactation, dehydration, renal failure.

Dosage and administration
Outwardly.The ointment is applied to the affected areas 4-6 times a day (as soon as possible after the start of infection). Ointment is applied either with a cotton swab or with clean hands to avoid additional infection of the affected areas. Therapy should be continued until the crust forms on the vesicles, or until they completely heal. Duration of treatment for at least 5 days, maximum 10 days.
Side effects
Hyperemia, dryness, peeling of the skin; burning, inflammation in contact with mucous membranes. Perhaps the development of allergic dermatitis.
Interaction with other drugs
The increased effect is noted with the simultaneous appointment of an immunostimulant.
special instructions
Ointment should not be applied to the mucous membranes of the mouth, nose, eyes and vagina. The effectiveness of treatment will be the higher, the earlier it is started (at the first signs of infection). To realize the therapeutic effect of acyclovir, the state of the immune system of the body matters. Patients with reduced immunity (with HIV / AIDS or after bone marrow transplantation) against the background of topical application of acyclovir ointment should be prescribed systemic administration of the drug, as well as in the case of severe and recurrent herpes infection. Acyclovir does not prevent the transmission of herpes through sexual intercourse; therefore, during treatment, it is necessary to refrain from sexual intercourse, even in the absence of clinical manifestations.
