Pharmacokinetics
Natamycin and neomycin are practically not absorbed through intact skin and mucous membranes. Absorption of neomycin through damaged skin is possible if the drug gets on wounds, ulcers. From intact to hydrocortisone is absorbed through intact skin; with dermatitis, the amount of absorbable hydrocortisone is increased by 2 times, with infectious skin lesions may increase up to 5 times. In children, the absorption of hydrocortisone through intact skin is higher than in adults, the degree of absorption of the drug into the systemic circulation decreases with age
Indications
Dermatosis, susceptible to GCS therapy, complicated by secondary bacterial and / or fungal infection (especially caused by fungi of the genus Candida).
Contraindications
Hypersensitivity to the drug; skin tuberculosis, skin manifestations of syphilis, viral infections of the skin, skin post-vaccination reactions, open wounds, ulcers; acne, rosacea, ichthyosis; anogenital itching, skin tumors. With care: tuberculosis (systemic lesion); children up to 1 year; in patients with a history of indications of defeat of the IX pair of cranial nerves against the background of the use of aminoglycosides; the presence of perforation of the eardrum and the direct effect of neomycin on the middle ear (there is a risk of ototoxicity).
Use during pregnancy and lactation
When prescribing the drug during pregnancy, it is necessary to take into account the theoretical risk of the ototoxic effect of neomycin, and therefore it is impossible to use the drug for a long time and apply it under occlusive dressings
Dosage and administration
Outwardly.For adults and children, the drug is applied to the affected skin 2-4 times a day. The duration of treatment is determined individually, taking into account the nature of the disease, the course of treatment usually does not exceed 14 days. In children, used under the supervision of a physician, the drug should be applied to limited areas of the skin, you can not apply occlusive dressings. In the form of a cream, the drug is recommended for the treatment of acute and subacute dermatoses, incl. accompanied by soiling. The ointment is intended for the treatment of sub-acute and chronic dermatoses, especially in the presence of severe dry skin, lichenification, seborrhea, or in cases where the occlusive properties of the ointment are necessary.
Side effects
Side effects from external use of Pimafucort, as with the use of other GCS, develop rarely and are reversible. Rarely, a hypersensitivity reaction (itching, burning, redness, or dry skin) may develop. Extremely rare - atrophy and thinning of the skin, telangiectasia, striae, purpura, rosacea-like and perioral dermatitis, delayed wound healing, depigmentation, hypertrichosis, withdrawal syndrome after stopping treatment. When using external corticosteroids for a long time, on large areas of the skin or when using occlusive dressings, especially in children, side effects characteristic of systemic corticosteroids, such as suppression of the function of the adrenal cortex, may develop. Contact dermatitis with neomycin.
Overdose
There is no information about drug overdose. There is a theoretical possibility of the ototoxic effect of neomycin in case of application of the drug to the area of the external auditory canal in the presence of perforation of the eardrum and the direct effect of neomycin on the middle ear.
Interaction with other drugs
There may be cross-hypersensitivity between neomycin, kanamycin, paromomycin, gentamicin; cross-resistance between neomycin and other aminoglycoside antibiotics.
special instructions
When applying Pimafucort on the area of the eyelids and periorbital area, there is a risk of increased intraocular pressure, development of cataracts. When using the drug in children on large surfaces or under dressings, it is necessary to keep in mind the possibility of suppressing the function of the hypothalamic-pituitary-adrenal system. In the case of prolonged treatment, with wounds and ulcers, there is a theoretical risk of the ototoxic and nephrotoxic effects of neomycin.If superinfection or excessive growth of fungi occurs, discontinue use of the drug and take appropriate measures.
