Pharmacological effect

Laxative. Causes a change in the flora of the colon (an increase in the number of lactobacilli), which leads to an increase in acidity in the lumen of the colon and stimulates its motility. Along with this, the volume increases and the softening of the feces occurs. As a result, a laxative effect develops, without direct influence on the mucous membrane and smooth muscles of the colon. Under the action of lactulose, ammonia is also absorbed in the colon, reducing the formation of nitrogen-containing toxic substances in its proximal section and, accordingly, their absorption into the vena cava system. It has the ability to inhibit the growth of Salmonella in the colon. Does not reduce the absorption of vitamins and is not addictive. Practically not absorbed from the intestines.

Pharmacokinetics

After ingestion lactulose by passage passes into the colon, where it is metabolized by bacteria that break down sugar. As a result, simple organic acids are formed — mainly lactic acid and, to a lesser extent, acetic and formic acid. A small amount of lactulose is absorbed and excreted in the urine unchanged.

Indications

Constipation, incl. chronic, during pregnancy, hemorrhoids, after surgical interventions on the colon and / or in the anus, to prepare for surgical interventions on the colon, to soften the stool (relief of defecation) in pain syndrome after removal of hemorrhoids, hepatic encephalopathy, hepatic precoma and coma.

Contraindications

Rectal bleeding, colo, ileostomy, suspected appendicitis, galactosemia, patients with lactose intolerance, lactase deficiency, glucose-galactose malabsorption, intestinal obstruction, hypersensitivity to lactulose.

Use during pregnancy and lactation

May be used during pregnancy and lactation (breastfeeding) according to indications.

Dosage and administration

The drug is intended for oral administration.The solution of lactulose can be taken both in diluted and undiluted form. It is necessary to immediately swallow the accepted single dose, without delaying the mouth. All dosages must be adjusted individually. In the case of a single daily dose, it must be taken at the same time, for example, during breakfast. During therapy with laxatives, it is recommended to take a sufficient amount of liquid (1.5-2 liters, which is 6-8 cups) per day. Dosage for treating constipation or to soften stools for medical purposes: The daily dose of lactulose can be taken once or by dividing it into two, using a measuring cup. The initial dose can be adjusted to a maintenance dose, depending on the response to taking the drug. The therapeutic effect may occur 2-3 days after the start of the drug. Dose for the treatment of hepatic encephalopathy (adults). Initial dose: 3-4 times a day, 30-45 ml. The safety and efficacy of the drug in children (up to 18 years) with hepatic encephalopathy has not been established due to lack of data. Elderly patients and patients with renal or hepatic insufficiency. There are no special recommendations for dosing, since the systemic effect of lactulose is insignificant.

Side effects

Interaction with other drugs

With simultaneous use with broad-spectrum antibiotics, it is possible to reduce the therapeutic efficacy of lactulose. With the simultaneous use of lactulose can disrupt the release of active substances from enteric preparations with a pH dependent release due to the fact that it lowers the pH of the intestinal contents.
